Scheduling an Appointment / Hours:
Hours: 7:00 am – 7:00 pm - Monday through Friday
(Some flexibility available)

You may schedule an appointment by calling our office directly, or your physician can schedule an appointment for you. Patients are asked to arrive 30 minutes early for their first appointment to complete necessary paperwork.

We now offer a printable new patient forms packet if you would prefer to complete your paperwork prior to arriving for your first scheduled appointment. Visit our Patient Information page for details.

What to bring:

  • Your written prescription
  • Nonrestrictive, comfortable clothing and footwear
  • Shorts are recommended for back or leg problems
  • Insurance information / insurance card
  • Completed New Patient Forms Packet (optional)
